About the business and the role

Care Forward is a locally owned and operated organisation, specialising in health and homecare services designed to support the independence of older Tasmanians and people living with chronic conditions or disabilities.

For 25 years we’ve been providing care and support to people in their homes across Tasmania. Our dedicated and passionate team includes nurses, allied health professionals and home independence support staff – all working to support and restore independence, health and wellbeing to thousands of Tasmanians across the State.

To support ongoing expansion we are needing to recruit additional Home Support Schedulers to our team based in our Rosny office. 

About You

You are an enthusiastic and dedicated individual  with a real passion for customer service delivered to the highest standard. You are adaptable, enjoy meeting new people and working in a variety of settings.

Job tasks and responsibilities include:

You will provide the scheduling of services for clients and the associated rostering of employees to deliver entry level community based supports and services.

  • Client supports.
  • Ensures the client Home Support is focused on person-centred service provision, enabling clients and their carers to continue to live in their community.
  • Applying a wellness philosophy to develop best practice, goal oriented, client directed supports. 
  • Assisting in the identification and brokerage of appropriate service delivery providers for the effective provision of services where needed.
  • Provide a customer focused telephone service to consumers and employees.
  • Maintaining electronically based filing systems and relevant data bases. 
  • Maintaining accurate and up to date consumer and employee records.

Preferred Experience

Experience as a worker in the aged care sector (residential or community setting).

Experience relevant to a customer/client service role.

Skills

Good verbal, written and interpersonal communication and relational skills.

Strong computer literacy with experience in word processing, calendar management software (Outlook) and data base applications including high level key board skills.

Ability to liaise and collaborate with service providers, referrers and consumers.

Mandatory Requirements

  • COVID-19 Vaccination Certificate (two doses plus 3rd booster)
  • Two (2) professional referee checks (2) – worked with candidate recently.
  • National Police Certificate obtained within the last three (3) month
