About the team The HOPE program is a Victorian Government initiative which was implemented to provide enhanced support and assertive outreach to individuals who have recently been discharged from an emergency department or medical ward following an attempted suicide, or serious planning or intent.
The HOPE program provide intensive, person-centred support with is tailored to the unique needs of the individual for up to 12 weeks. These individuals are supported to address factors that contribute to stress in their lives, which can include assistance with employment, mental health support, and referral to range of other community supports. The HOPE program also works with the families, carers, or friends of the individuals who have attempted suicide, to provide support and link them in with appropriate services.
About the role An exciting opportunity exists for motivated Mental Health Clinician to undertake an ongoing position within the HOPE team working from the Ballarat region.
The successful applicant would have the ability and desire to provide patient-centred, recovery minded, psychosocial focused support to the individuals and their families, carers, and friends, post an attempted suicide or serious planning or intent. Other key expectations include.
Provide comprehensive mental health assessment and treatment plans to people referred to HOPE program for secondary consultation or intake into the 12-week psychosocial program.
Provide evidence-based interventions for consumers post suicide attempt, engaging in significant deliberate self-harm, or voicing suicidal ideation.
Share skills and knowledge with key groups, formally and informally, to strengthen and enhance existing programs and clinical responses to people who may present with risk factors for suicide.
Develop expert knowledge in the field of suicide prevention, including psychiatric, psychosocial broader community-wide evidence-based approaches to reducing suicide rates within the Grampians region.
Skills and experience
Relevant professional qualification in a health-related discipline (psychology, nursing, social work, or occupational therapy); current registration where applicable.
Experience required is relevant to the respective enterprise agreements.
