ABOUT THE ROLE:
The Individual Support Manager role is a Full-Time Position based at our Regional Office in Bathurst NSW.
As the Individual Support Manager, your main purpose is to coordinate and implement Individual Support services to people with acquired brain injuries within a team of other Individual Support Managers. You will work with clients to deliver Individual Support to meet their needs, build their skills and connect them with their community, so they can achieve their personal goals.
Key Responsibilities
- Develop, implement and monitor client Individual Support plans, including activities of daily living, community engagement, and rehabilitation, within the client funding allocations
- Collaborate with clients, families, healthcare professionals, and other stakeholders to implement Individual Support plans
- Supervise, support, and mentor Community Support Workers to deliver quality support and meet our clients’ individual needs
- Assist with recruitment, training, supervision, competency assessment and support of Community Support Workers.
- Create and maintain fortnightly rosters, including filling shifts when required and approving shift attendance
- Provide 24 hours On-Call support and crisis management Monday to Friday each week and every ninth weekend.
- Maintain accurate and up-to-date records of client support, documenting progress and any changes in their needs.
- Working collaboratively with other Individual Support Managers.
- Adhere to our quality management system and standards.
- Adhere to policies and procedures
- Promote independence, choice and control to the Client to achieve their individual goals.
- Ensure that clients’ rights are respected and that they receive support in line with their cultural and personal preferences.
- Liaising with funding bodies where appropriate
Essential requirements
- Driver Licence and Vehicle Registration
- First Aid Certificate
- NDIS Worker Check (NDISWC)
- NDIS Worker Orientation Module
Qualifications
- An appropriate diploma or tertiary qualification relevant to the work required to be performed.
- Previous experience in a relevant industry or service to undertake the range of activities required.
Essential skills or experience
- Client Coordination and management within client funding arrangements and Individual Support
- Supervision of Support Staff including recruitment, training, supervision and competency checks to deliver Individual Support to clients
- Highly developed communication and relationship building skills with all levels of staff and external providers
- Staff Roster allocation and management
- Crisis management and on call support
- Ability to take initiative and be self-directed
- Ability to work as part of a team
- Extensive experience in use of computers and client management systems to ensure documentation and maintenance of client records are adhered to
- Ability to set priorities, monitor workflows and meet timelines
- Ability to promote independence, choice and control to support Clients to achieve their individual goals.
- Ability to monitor client wellbeing, manage changes and respond flexibly and creatively to ensure clients support is always relevant to their individual needs.
- Ability to build client capacity within the context of their support and rehabilitation goals
- Experience working with external Service Providers and Allied Health Professionals on client Individual Support plans.
- Experience maintaining accurate records.
- Experience to adhere to quality management system and standards.
- Experience adhering to policies and procedures.
