Job Description:
We are currently seeking Key Workers to join our team at My Ability Pathway (MAP). As a Key Worker, you will be responsible for supporting the day-to-day running of our Supported Independent Living (SIL) services or our community support services. Catering to clients with varying types of disabilities within the guidelines of the National Disability Insurance Scheme (NDIS).
The Key Worker role is an operational position that supports the delivery of services to our clients. The Key Worker will work collectively with the Team Leader, undertaking shifts, mentoring and supporting their team, carrying out administrative duties and playing an integral role in the daily planning and implementation of support. The Key Worker is expected to undertake all aspect of disability support including personal care and shifts across a 24/7 roster.
Responsibilities:
- Work closely with clients to understand their specific needs, goals, and aspirations outlined by the NDIS.
- Collaborate with Team Leader (TL) to ensure smooth operations and service continuity.
- Support clients in achieving their personal goals related to healthy lifestyle factors, preventative health care, and overall well-being.
- Share relevant client information with staff members while maintaining confidentiality.
- Work collectively with the TL to manage staff. This will include weekly staff check-ins and priorities, documentation and communication about staff performance, mentoring and guiding the teams in professional development, performance excellence and success planning.
- Develop, implement, and commit to best practice policies and procedures.
- Foster positive relationships with clients, staff members, families/carers involved in care plans.

Qualifications:
- Certificate III in Individual Support (Disability) or equivalent qualification required; higher education preferred but not mandatory
- Previous experience or knowledge about disability support services.
- Current First Aid Certificate with CPR
- National Police Clearance (no older than 6 months)
- Drivers Licence
