MDSS Wellbeing Practitioner

Moranbah and District Support Services Inc (MDSS)

Isaac Region (Moranbah, Glenden, Clermont, Coppabella, Dysart, Middlemount and Nebo)

Full-time position (pending applicants qualifications and experience)

MDSS is a not-for-profit community service organisation providing vital support services to the Isaac region communities include a Neighbourhood Centre Program, Rural Family Support Service, and Isaac Wellbeing Project. 

About the role

The MDSS Wellbeing Practitioner position will provide support to all members of our community who need support with mental health, domestic violence, or community education. The role also provides information, advocacy, advice, and referral to assist clients gain access to services. 

This role will require regular travel to outreach communities within the Isaac Region. 

Key Position Responsibilities

  • Providing short term interventional counselling to vulnerable children, young people and families, extended family members and carers, in relation to issues impacting on the well-being and safety of children and young people.
  • Providing information, advocacy, advice and referral to assist clients gain access to needed services.
  • Developing and maintaining appropriate referral networks, collaborative partnerships and positive relationships with other service providers and Government departments across the region and State to support the communities in the Isaac region.
  • Developing and delivering an evidenced based, best practice case management model for MDSS service users.
  • Actively participating in the case management with clients by developing and implementing an overall case plan, session breakdown and review processes.
  • Ensuring processes are proactive and focused on achieving outcomes identified by the client.
  • Using a variety of appropriate strategies to engage difficult to reach cohorts including Aboriginal, Torres Strait Islander or Australian South Sea Islander, and people from culturally and linguistically diverse backgrounds.
  • Actively developing, promoting, and participating in community programs that support MDSS' goals and mission within the community.

This role will require travel to outreach communities and may require overnight stays.

  • Work co-operatively as a team member, and autonomously as required, to achieve smooth and effective functioning of MDSS, and to achieve program and organisational goals.
  • Maintain a high level of community contact to enhance the profile of MDSS. Represent and actively promote the service at meetings, public forums, in the media and at other networking opportunities. 
  • Maintain strict confidentiality in regard to all client and work issues.
  • Undertake and participate in Professional Development, training and organisational planning and evaluation activities as determined appropriate to the needs of the worker and to meet the goals and objectives of MDSS.
  • Actively participate in your own Performance Appraisal process as determined by the Management Committee
  • Comply with all policies and procedures established by the service.
  • Promote and maintain MDSS Inc mission, vision, and goals.
  • Undertake other such duties as may be directed by the Management Committee.

Responsibilities

The successful applicant will have a friendly, community focused attitude, and be able to work autonomously and as part of a small team. They will possess high level communication skills and time management skills and have a commitment to client confidentiality. This position requires data collection and presenting verbal and written reports on all aspects of service delivery, and annual performance appraisals.
