NDIS Evidence and Access Co-ordinator

Location: Derby, Western Australia

Employment Type: Full Time

Remuneration: $69,435.08 - $86,793.85 per annum pro rata + superannuation + salary sacrifice

The NDIS Access and Evidence Co-ordinator will assist people with disabilities to test eligibility to the National Disability Insurance Scheme (NDIS), navigate the NDIS access process and to assist with coordination of the Scheme’s plan development process.

This will include gathering of information and evidence required for eligibility/access and if access eligibility requirements are met the participant will be supported through the coordination of planning meetings between the NDIA and the participant. If the NDIS access eligibility requirements are not met the person will be connected with other services, supports or pathways.

About You

To be considered for this position, you must have:

  • Experience of working with families with complex needs, within the social services sector.
  • Ability to work with Government and non-government agencies.
  • Experience with health or social service delivery.
  • Demonstrated experience working with Aboriginal people from the region, especially within a remote setting.
  • Provide strategic direction to work area, including developing strategies and priorities and monitor work flow and performance.
  • Effective communication and people engagement skills and ability to develop relationships with marginalised and at-risk families.
  • Demonstrated good communication skills, including computer skills.
  • Current WA Class C driver’s license.
  • Participate in cross cultural education or staff who are not familiar with Aboriginal customs and traditions.
  • National Police clearances and Working with Children Check.
  • Willing to undertake a NDIS Worker Screening Check
  • Fully Covid-19 Vaccinated

Ideally, in addition to the above, we are looking for someone who has:

  • First Aid Certificate.
  • Knowledge of NDIS

The position involves working within a multicultural organisation where the majority of employees, clients and stakeholders identify as Aboriginal and/or Torres Strait Islander. Candidates must be able to demonstrate an understanding of the issues affecting Aboriginal and Torres Strait Islander people and their ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people.

About the Organisation

Derby Aboriginal Health Service (DAHS) is an Aboriginal Community Controlled Health Service (ACCHS) providing holistic primary health care services to the Aboriginal and Torres Strait Islander people living in Derby and surrounding communities. DAHS provide a wide range of innovative public health programs in addition to direct comprehensive primary health care services. It is required to comply with a number of Service Agreements with Commonwealth and State departments and agencies, and to operate with a number of government and private medical and health delivery organisations. The operating environment for ACCHS Australia-wide - has become increasingly complex and changeable in recent years and this is likely to continue.

Key Responsibilities for this position are:

The scope of this position will include, but is not limited to:

Programs

• Act as liaison and planning support person for families using the service.

• Facilitate and establish relationships with people with disability and families and service providers in community working in and alongside (as relevant), remote community connector and NDIA staff.

• Ensure cultural safety is incorporated into the service delivery model.

• Maintain and update client files to record daily service activities and the services clients have accessed.

• Facilitate the access process via identification and collation of documented information and evidence requirements working in and alongside (as relevant), remote community connector and NDIA staff.

• Facilitate and coordinate meetings and sessions between participant/s, relevant stakeholders, remote community connector and NDIA staff that address access requirements.

• Facilitate and coordinate local linkages with DAHS, community school and other community based service organisations working in and alongside (as relevant), remote community connector and NDIA staff.

• Perform other duties as required by management.

• Work with Government and non-government agencies effectively to support program participants.

• Develop knowledge and skills that support the program capacity to deliver quality services

• Agree to and abide by relevant professional ethics and client confidentiality.

Develop and Maintain Linkages

• Make recommendations and assist in formulating new policy within DAHS.

• Liaise and cooperate as required with other Project and Policy staff in DAHS.

• Promote positive public relations with patient’s, peers, government and non-government agencies and services to achieve equitable outcomes for participants.

• Ability to positively work as part of a team.

• Ability to work with people with complex cultural and social needs.

• Ability to work with people with challenging behavioural issues.

Culturally Appropriate Ways of Working

• Personally demonstrate through consistent behaviour understanding of, respect for and compliance with culturally sensitive ways of working with Aboriginal and Torres Strait Islander clients, staff and communities.

• An understanding of local Aboriginal cultural, social and economic issues impacting on Aboriginal people.

• Ensure cultural appropriateness of resources for organisational use.

Other

• Participates in continuous quality improvement activities and applies quality improvement principles to all duties performed. Attends
