About the role
This position will form a part of the multidisciplinary Integrated Care (IC) Team to provide culturally appropriate comprehensive assessment, clinical intervention and care coordination. Integrated Care is a growing and exciting area to work, serving as an important conduit between primary care and hospital specialty services.
The role will promote and advocate a ‘home first’ approach, where multidisciplinary assessment and intervention will be delivered outside the acute hospital setting. The Senior Occupational Therapist will be required to work across CALHN sites and within Integrated Care, as required and in line with clinical need. This includes Hospital in the Home (HITH), Refugee Health Service (RHS), Hospital Avoidance clinics and direct referrals from other CALHN sites.
The role will focus on the assessment and management of clients with complex health needs who are experiencing gaps in health responses and will provide short-term intervention and facilitation of care transition to acute, sub-acute and community services. Occupational therapists play an important role as the ‘in-reach’ arm of our service. This offers the opportunity to review patients in the context of their home/care environment and support them with short term intervention and effective linkage to services that can meet their ongoing needs.
The Senior Occupational Therapist supports the Team Manager in providing clinical and professional leadership for the Occupational Therapists and helps to inform service priorities and opportunities.
Allied health is a key component of our 7 day service model. Contracts for successful incumbents will be aligned to 7-day rosters, although at this time our Occupational Therapists are not required to work weekends.
Integrated Care is committed to embedding virtual care / telehealth alongside traditional methods of consultation (face to face or phone).
Qualifications
- Appropriate degree or equivalent which entitles registration as an Occupational Therapist by the Occupational Therapy Board of Australia (OTBA).
