Southern Adelaide Local Health Network is seeking a passionate, talented, and caring Occupational Therapist or Social Worker to join our dynamic team.
This role will be primarily involved the discharge planning for patients of Transition Services, coordination of client Services and provision of either Social Work or Occupational Therapy assessment and intervention.
Please note: These positions are multi-classed as either a Social Worker or Occupational Therapist and classification will be determined by qualifications/experience.
About the Role:
Reporting to the Clinical Manager in Transition Services, your duties may include:
- Developing, implementing and evaluating innovative responses and flexible service options to meet identified needs to support the Transition Services.
- Clear and accurate reports, presentations and contribution to publications regarding the program operation, outcomes and its objectives.
- Working cooperatively within a multi-disciplinary team to continually improve, implement and evaluate a holistic Transition Services which is responsive to identified community needs.
- Providing written information to staff, patients and family/carers regarding the Transition Services.

About you:
(Occupational Therapist)
- A degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
- Demonstrated ability to assume responsibility for the provision of clinical Occupational Therapy services to a designated area as a member of an interdisciplinary team.
- Demonstrated skills in Occupational Therapy assessment and intervention, in particular related to older people
(Social Worker)
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Demonstrated skills in case management/care coordination and the ability to make complex clinical decisions including sourcing and coordinating services to meet clinical and community care needs.
- Sound skills in building rapport and communicating with a wide range older adults, their carers /family, referrers, hospital staff and service providers
