What You Will Be Doing
The Outreach Homelessness nurse works in the Homelessness and Health Outreach Service, providing case management and assertive outreach to people who are homeless, living in low cost or insecure housing in the LGA areas of Frankston and the Mornington Peninsula Shire. The aim of the M.I. Health program is to link people from diverse backgrounds into health, housing and welfare services.
What You Need
We anticipate you will have:
• Registered Nurse (Division 1) with APHRA registration.
• Experience in case management or similar modalities.
• Experience working in health care, community health or outreach settings.
• Current Driver’s License.

Sub-grading will be based on years of experience.
