About the role

Pain Rehabilitation Clinic Coordinator is responsible for coordinating a multidisciplinary team to ensure the effective establishment and ongoing operations of the Pain Rehabilitation Clinic at South West Healthcare (SWH).

The Coordinator will lead and advise a multidisciplinary team to ensure a biopsychosocial model of person centered care for clients living with chronic pain. The role is responsible for compliance and reporting in line with relevant legislation, policy directives and funding agreements.

The Coordinator will ensure the Pain Rehabilitation Clinic operates in an efficient manner with appropriate referral pathways and is compliant with quality standards. Stakeholder and consumer engagement will be maintained to ensure the program meets the needs of people with chronic pain in the South West region.

Position overview

  • Part-time (32 hours per fortnight), On-going position
  • Warrnambool Campus
  • Community Health Nurse, Physiotherapist or Occupational Therapist Grade 2
  • Salary Range: $86,730.80 - $102,278.80 per annum pro rata, plus superannuation

What you bring

You're proactive, flexible, and resilient and happy to work autonomously, use your initiative, and have a high attention to detail.

You will have an ability to comprehensively assess, plan, implement and evaluate clinical care of the client with chronic and persistent pain.

You will build and maintain effective relationships with a multidisciplinary team, clients and other stakeholders, including participation in relevant networks.

You will be a Registered Nurse, Physiotherapist or Occupational Therapist that holds a current practicing certificate with the Australian Health Practitioner Regulation Agency (APHRA) with a minimum of 3 years of clinical experience.
