About Kiind 

Kiind is a family-led, independent peer support organisation with almost 40 years of operation. We support 6,400+ Western Australian families raising children living with disability, developmental delay, autism, genetic, rare, undiagnosed and/or chronic conditions. 

Based at Perth Children’s Hospital (PCH), our experienced Peer Navigation team have extensive knowledge of the health, disability and education systems and help families to find the answers they’re looking for to best support the needs of their child. We have a range of information resources to help with this and we also connect families to one another. 

Through active systemic advocacy, we help decision makers to understand the experience of children with disability and their families. We seek to influence policy, legislation, and service design.

All our services are provided free-of-charge.

About the role

Kiind is seeking a Peer Navigator to join our vibrant, up-beat team, located in Nedlands. In this role, you will be the first point of contact for families with children who may be inpatients, outpatients visiting PCH for clinical appointments, as well as nursing staff and other health professionals with whom we work to provide holistic care to families.

This role is 0.8 FTE (4 days per week) and will be offered on a contract until 30 June 2025. 

In this role, you will be required to: 

  • Provide support and information on Kiind services and resources to prospective and current members 
  • Conduct ward visits with inpatient families at PCH to offer resources, care items and support
  • Provide NDIS support in areas such as pre-planning, navigation and Plan review to help build capacity for families so they better understand and are able to effectively implement their child’s NDIS Plans
  • Conduct Pathway Planning sessions with families and follow up with links to support and resources
  • Liaise with families in the Kiind centre at PCH, by phone, video call, email and web enquiry
  • Welcome and onboard new Kiind members
  • Connect Kiind members via Kiind’s Coaching Program
  • Online peer support group facilitation as required 
  • Maintain relevant case notes in the Customer Relationship Management (CRM) system
  • Continual mapping of systems and services for families 
  • Peer review of Kiind resources as required
  • Reception duties including answering phones, taking messages and scheduling appointments
  • Monthly operational reporting 

What you’ll need to succeed

  • Lived experience of raising a child with disability, genetic, or chronic medical condition
  • A warm and positive personality with the ability to support anxious or distressed parents, build rapport and develop long-term, productive relationships
  • Excellent knowledge of the NDIS, disability services and community support
  • Strong ability to research and find information for systems, services and supports mapping
  • Goal-focused with ability to problem-solve and help families build a road map to positive outcomes 
  • Competency with Microsoft Office Suite 
  • Excellent spoken and written communications skills
