ABOUT THE ROLE

We are seeking an experienced Post School Transition Coordinator to work 15 hours per week (term-time only) preferably working on a Monday and Tuesday, at our Wahroonga campus.

The role of the Post School Transition Coordinator is to support students, their families, and the school with the transition planning process to Post School opportunities.  The Post School Transition Coordinator will gather information relating to the student’s Post School goals, pathways, and support needs and connect the students and their family with appropriate services and community groups. The role will predominantly focus on working with students and their families in Years 10, 11 and 12.

KEY RESPONSIBILITIES

  • Support families to understand possible Post School pathways consisting of finding a job, becoming work ready and ready to find a job, study and work (e.g. apprenticeships, traineeships), community and recreational participation and social activities, increasing independence and home living.
  • Engage in a collaborative transition planning process for the short and medium to long-term with students from Years 10 – 12 and their families. Goals will be considered in the following areas; Further Education and Training, Employment, Personal Care and Management, Health and Wellbeing, Social and Community Participation, Transport and Mobility.
  • Schedule collaborative transition planning meetings with the student, their family and relevant school staff including the student’s class teacher and service providers where relevant.
  • Gather relevant information to support successful transition planning to Post School destinations including current supports, IEPS, School reports, VET records, student profiles and interest profiles, work experience and work placement information.
  • Gather information for NDIS access meetings or plan reviews which can be shared with NDIA with the families consent.
  • Complete an Individual Transition Plan in collaboration with the student, parent and carer, other school staff, and providers as appropriate. The transition plan can be used by parents to present to NDIS Local Area Coordinators and planners to inform NDIS planning decisions.
  • Support students and their families to share information with the NDIS, employment service providers, or other disability support providers as they choose.
  • Collaborate with the Family Support Worker to build up a directory of services in student’s local area.
  • Develop and maintain positive relationships between the School and service supports and community groups.

  • The successful applicant could also hold qualifications in education, health promotion, disability, or allied health. 
  • An understanding of the potential needs of people with a disability, their families and carers, including those students with complex needs.
  • Demonstrated understanding of person-centred planning and a commitment to capacity building for young people.
  • Solutions focused approach is essential.
  • Excellent interpersonal skills and communication skills, with demonstrated ability to build rapport and interact effectively at all levels.
  • Capacity to stay self-regulated in situations of high stress and problem-solve at these times.
  • Demonstrated experience of working collaboratively within a team.
  • High level of integrity and trust, professional demeanor, understanding of the importance of confidential information.
  • Excellent written and oral communication skills, great organisational skills and attention to detail.
  • Current Australian Driver’s License and current and verified Working with Children Check.
  • Proficiency in Microsoft Outlook and Microsoft Word.
  • First Aid qualifications.
