Primary Health CoordinatorFraser Coast | Bundaberg / Wide Bay Region

Provide local leadership of Country to Coast Queensland (CCQ) stakeholder relationships, partnerships and alliances with Primary Healthcare providers.

CCQ delivers the Primary Health network (PHN) program for Central Queensland, Wide Bay and Sunshine Coast on behalf of the Commonwealth Department of Health.  CCQ has an ambitious agenda to design and deliver better Primary Health outcomes for the people in their region. Achieving this will require CCQ as a commissioning organisation to not only focus on Primary Health service provision but also on the environmental, social and cultural determinants of health which support communities to live their best lives.

CCQ’s diverse region covers 161,108 km2 from Sunshine Coast to Rockhampton and west to Emerald. The population in the region is just under 1M people with around 68% living regionally or remotely and more than 28% living in the most disadvantaged quintile. CCQ collaborates with the community to provide exceptional Primary Healthcare services to reduce health inequalities for the region. CCQ has seven key priorities for health improvement: mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health and older person’s health.

The Wide Bay is a stunning location to live in with beautiful beaches and surrounding bushland for getting out into nature, it is also home to the UNESCO World Heritage-listed Island K’gari (formerly Fraser Island) and the Southern Great Barrier Reef.

The Wide Bay is a beautiful place to live, along with a stunning coastline and bushland, it has a wonderful agricultural history providing fresh food including avocados, macadamias, and fresh vegetables. It offers a relaxed and family friendly environment to live and work. Our larger towns like Bundaberg, Hervey Bay and Maryborough have great local health and education facilities while still offering small town charm. There is something for everyone in the Wide Bay.

About the role

  • The Primary Health Coordinator will be based in the Wide Bay and will provide local leadership of Country to Coast Queensland (CCQ) stakeholder relationships, partnerships and alliances with Primary Healthcare providers, Hospital and Health Services, community services, and social services in communities.
  • The role is responsible for the successful local implementation of a range of programs, projects and initiatives designed to drive high-performing, coordinated service delivery and person-centred healthcare.
  • Responsibilities include working collaboratively with general practice and other key stakeholders, using data and information to underpin engagement with local Primary Health providers, implementing health system reform and supporting local initiatives throughout the region.

The successful person will work with diverse external stakeholders, build and develop relationships, and implement health system reform and local initiatives. This will also involve using data and project management skills to implement health system reform and support organisational initiatives across the region.
