Right at Home – Sydney Lower North Shore provides high quality in-home care to the elderly and those living with a disability who require assistance in their home. We also provide after hospital care. We require a passionate and dedicated Care Coordinator to join our office to assist in the performance of the co-ordination of in-home care. Due to our demographics we require a Care Coordinator who is a Registered Nurse with English & Chinese speaking skills. This will be a part time position with expected growth over time.
Responsibilities will include:
- Dealing with a range of disabled, seniors and frail elderly customers and their families daily, this role will handle anything from information requests, feedback, general care enquiries to complaints.
- Coordinate, organize and administer in-home healthcare programs and clinical services: nursing care, rehabilitation & allied health, and non- clinical support services.
Essential Criteria
- Appropriate qualifications and experience as an office/care/ manager
- Australian Federal Police Clearance Certificate
- CPR and First Aid certificate
- Knowledge of the Aged Care and Community Care Sector, Continuous Quality Improvement, legislation, Common Care Standards, and Community Care Charter of rights and Responsibilities.
- Driver’s License and comprehensively insured dependable private vehicle
