The Program Leader Healthy Communities is responsible for providing effective operational management and leadership to staff as they provide a broad range of primary health care, health promotion, and community support services to the people and communities Grampians Community Health serves. This position will entail travelling between GCH sites.
Programs within Healthy Communities currently include Health Promotion, Community Health Nursing, (including general, women’s health, movement disorder, diabetes and chronic conditions) Freeza, School Focused Youth Service, Healthy Mothers- Health Babies and Diabetes Case Management.
As a key member of the leadership team, the Program Leader Healthy Communities is part of a cohort committed to developing a company culture focussed on excellence in service standards, continuous improvement and quality assurance.
The position will contribute to achieving the vision, goals and values articulated in the organisation’s strategic plan.
