Moyne Health Services in Port Fairy invites applications from experienced Registered Nurses to join the Community Health team as a District Nurse on a casual basis.
About the Role
The primary objective of the District Nursing role is to provide a person centred approach to assessment and care, whilst attending to clinical needs and assisting clients to maintain their independence.
Healthcare interventions for home based clients may include:
- Clinical nursing assessments and practices.
- Developing, implementing and monitoring client goal directed care plans
- Providing health management, education and information
- Monitoring the person's health status
- Supporting carers and the care relationship
- Personal care for people with unstable health and/or complex needs
- Coordinating nursing and health services with other service providers
- Facilitating referral to other key agencies.
- Liaising with other services to promote a collaborative team approach to care.
About You
Minimum Qualifications
- Bachelor of Nursing with current AHPRA registration
Key Selection Criteria
- Minimum 3 years' experience across a broad range of clinical
settings - Knowledge of the wellness and re-ablement approach to aged care
with regard to goal setting and care planning. - Knowledge of issues relating to frail / aged, people with disabilities
and their carers - Understanding of Accreditation Standards, quality systems and
continuous improvement models - Working knowledge of My Aged Care (MAC), Commonwealth Home
Support Program (CHSP) and Home and Community Care program
(HACC), Home Care Packages (HCP), National Disability Insurance
Scheme (NDIS), Post-Acute Care (PAC), Hospital in The
Home(HITH) - Demonstrated written and verbal communication skills
- Computer literacy
