Role Overview:
Join us as a Registered Nurse, pivotal in delivering safe, effective nursing care to Community Aged Care clients. Work closely with our Clinical Nurse Specialist and multidisciplinary team, being part of a highly motivated and inclusive team, to make a real difference. Key Responsibilities:
- Deliver quality clinical care under the direction of the Clinical Nurse Specialist.
- Undertake or delegate clinical assessments for clients
- Develop nursing care plans with SMART goals in collaboration with clients and carers.
- Use a consumer-focused approach to support the organisation’s commitment to a “Great Client Experience.”
- Consider social, cultural, psychological, physical, spiritual, and environmental aspects in assessments.
- Identify and respond to deterioration and/or changes in a consumer’s condition in collaboration with the multi-disciplinary team and other health care providers and support services
- Ensure infection control practices are followed by the team.
- Maintain timely, accurate, and relevant nursing documentation.
Skills & Experience:
- Recent experience as a Registered Nurse (RN) in community aged care.
- Provide direct clinical care, support, leadership, and education to clients and staff.
- Work autonomously and within a multidisciplinary team.
- Make evidence-based clinical decisions and problem-solve.
- Excellent time management skills.
- Sound clinical nursing knowledge and confident decision-maker.
- Educate consumers and their families about clinical care.
- Communicate clinical recommendations effectively.
- Communicate sensitively and openly with consumers, families, and staff.
- Understanding of Aged Care Quality Standards.
- Knowledge of community aged care funding and home support programs
Essential Requirements
- Bachelor of Nursing
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Current First Aid Certificate
- Evidence of ongoing clinical professional development
- Current DHS NDIS Worker Screening Check
- Current SA driver’s licence
- Reliable vehicle and comprehensive vehicle insurance
