Registered Nurse Care Coordinator

Vitalis invites Registered Nurses with Orthopedic and Cardiac experience to join our wonderful Telehealth team in St Leonards, NSW (2065). 

This is an exciting time to join our established Discharge Support Services team to make a positive difference in peoples' lives.

• Rewarding role as part of Vitalis' highly valued telephonic clinical team.
• Work ordinary hours 8:30am-5pm.
• Part-time position: 2 or 3 days a week

Discharge Support Services (DSS) is one of Vitalis’ specialised programs. This service is delivered by a team of specialist clinical nurses specifically trained to provide phone based, patient-centered discharge support and care coordination services. 

Selection Criteria

• Minimum 7 years’ experience as a Registered Nurse
• Current or recent experience in working in Orthopedics and Cardiology 
• Experience using computers and related software
• Excellent nursing assessment skills
• Strong critical thinking and decision-making ability
• Effective verbal & written communications skills – fluent English is essential.
