Your role:
We currently have an exciting opportunity for an experienced CDM/GP Nurse for a 12-month fixed term contract in our Armadale Practice. The ideal individual will have a passion for providing supportive and patient focused clinical care within a primary health care setting, focusing on improving health outcomes for our patients. This position would suit an individual who enjoys wearing multiple hats throughout the week and thrive off diversity and change, working in both Chronic disease management and GP Nursing environment.
What you will be doing:
General Practice Nursing
Performing health assessments and chronic disease care planning based on best practice principles
Focusing on preventative health strategies to help reduce lifestyle risk factors
Maintaining and developing new working relationships with independent and interdependent members of the medical, nursing and allied health professionals to value add to our patient's health journey
Performing ECG, Spirometry and Ankle Brachial Index as appropriate
Some clinical tasks you will be performing will include but not limited to ear syringing / dressings / de-sensitisations basically, all GP nursing treatment tasks
To join you will have:
Registration with AHPRA - Registered Nurse or Enrolled Nurse with no restrictions
Experience working in a GP Practice
Understanding Vaccines and the National Immunisation Program Certificate (essential)
Chronic Disease Management experience
Some knowledge of 721's 723's and Health Assessments
Current CPR certificate
Knowledge and familiarity of Best Practice software (highly regarded)
Police Clearance (inside 3 months old)
