- Responsible for patient Home Dialysis education and ongoing support
- The Home Dialysis Service – RMH Kidney Care Service (KCS)
- The RMH Kidney Care Home Dialysis Service (HDS) provides dialysis care to patients and families in the northern and western suburbs of Melbourne and areas throughout regional Victoria.
- The HDS team embraces a leadership role in evolving services to promote independence and well-being for people who choose the options and benefits of home haemodialysis or peritoneal dialysis.

- The role of the Home Dialysis Services nurse is the provision of patient and family oriented renal services that promote independence in the home setting.
- Responsible for patient education in home haemodialysis and peritoneal dialysis, including PD and Home Haemodialysis training in the home.
- Participate in outreach nurse clinics and home visits in metropolitan and rural areas.
- Facilitate the coordination of pre-dialysis and post-education care via face to face or telephone consultation
- Assume responsibility for primary patient caseload management.
- The nurse in this position will be an active member of the Home Dialysis team and will contribute to quality improvement in all areas of service delivery
- Partner with people leaders to provide contemporary, innovative and accurate best practice advice.
- Ensure continuous improvement to achieve high quality of patient care outcomes and meet organisational goals.
- Work within the team and support others to deliver excellent nursing practice and care within the defined scope of practice.
- Registered nurse division 1 with current registration with AHPRA
- Post-graduate qualification in renal nursing and minimum 2 years of peritoneal dialysis and haemodialysis nursing experience
- Excellent communication and interpersonal skills
- Ability to be innovative and resourceful
- Ability to adapt best practice health care models into professional nursing practice
- Patient outcome and team focus
- Experience in patient education
- Current Australian Driver’s license
- You are an AHPRA registered Nurse with ward experience and are willing to work on a rotating roster, within our supportive and dedicated multidisciplinary team.
