Our Access Aged Care team is providing co-managed primary care services to residents in Aged Care facilities throughout Australia.Your New RoleReporting to the Nursing Regional Manager, this is a great opportunity to join our growing organisation in a newly created position focused on optimising health outcomes for our customers.In this integral position, you will provide clinical management and coordination and will partner with residential aged-care facilities. You will be a motivated and passionate individual who can work well in an autonomous role and appreciate the freedom and flexibility this role offers. Working onsite at region-specific aged care facilities you will also regularly travel to partnering homes to facilitate clinical reviews amongst other responsibilities. This position will be situated in and around Bendigo but travel to other areas may be required. The Registered or Enrolled Nurse will work as part of a multi-disciplinary team to ensure efficient and high-quality service delivery coordinating the clinical team with the support of the Nursing Regional Manager and administration team.
