AddressRural Health Tasmania is seeking experienced Enrolled Nurses and Registered Nurses to work within our North-West Services. The position is 4-5 days per week, either part time or full time as negotiated.
The successful applicant will provide chronic disease management services under the Rural Health Tasmania integrated, coordinated stepped care model to address Mental Illness (child, adolescent, adult), Cancer, Circulatory Disease including CVD, Respiratory Disease including COPD, Musculoskeletal conditions and injury in Sheffield.
REQUIREMENTS Essential
- Sound understanding of supporting and coordinating care for people with chronic conditions
- Registration with the Nursing and Midwifery Board of Australia or AHPRA.
- Sound understanding of social determinants of health
- Working with Vulnerable People Registration and National Police Check
- Current Drivers Licence
