You would be expected to provide person-centred care to your Clients in the community, ensuring a safe environment within the facility and applying your clinical skills to improving their quality of life through activities that include:
• Assess, plan and implement client care within your scope
• Consider physical, social and psychological requirements
• Create and update care plans to reflect developing needs
• Ensure clinical quality compliance standards are met
• Work collaboratively with other multi-disciplinary team members
This is mobile role where you will be required to drive to complete your scheduled community visits.
Your key attributes
• Excellent communication and clinical problem-solving skills
• Effective time management, and able to work autonomously.
• General nursing experience, post-registration
• NDIS Workers Screening Clearance or willingness to obtain
• Full, unrestricted AHPRA registration and unlimited work entitlements
• Current Australian Driver’s Licence
