The Access Aged Care team offers co-managed primary care services to Aged Care residents across Australia.Your New RoleStep into an exciting full-time position reporting to the Nursing Regional Manager, contributing to our flourishing organization. This role focuses on improving the health outcomes of our valued customers. Spearheading clinical management and coordination efforts in collaboration with aged-care facilities. We seek an autonomous, motivated individual based in Gosford, with occasional travel to other areas.The Registered or Enrolled Nurse will work in a multi-disciplinary team, ensuring efficient service delivery. Contribute to business development and expand the resident base.
