Relieving Care Manager
Registered nurses with excellent clinical skills and care management experience to cover Care Manager leave and also to assist with clinical improvement projects at Aged Care facilities across South West Vic, North Vic, South East Metro Vic. There is also potential to travel interstate to WA, NSW and QLD.
Depending on location, applicants may be required to stay away from home on occasions while covering Care Manager leave. Travel and accommodation costs will be covered by the organisation.
The Role:
Supervise staff in respect of the quality of care provided to residents with a person centred approach.
Lead by example and act as a role model to the staff.
Work well in a multidisciplinary team and provide resident focused care ensuring to report any issues to the relevant parties.
Assist homes with assessment and care planning responsibilities.
Provide clinical leadership to homes, in the delivery of care, monitoring of clinical systems and in addressing practice and process deficits.
Provide coverage for Care Managers during periods of leave.
Requirements:
Registration with AHPRA as a Registered Nurse Division 1
Minimum of 3 years’ experience as care management experience
Working knowledge of the aged care standards
The ability to lead practice change through role modelling and education
High level of interpersonal and communication skills (written and verbal)
Demonstrated understanding of person-centred care and ability to work in a team environment
