• Rostered position - 7 days. Flexible start times.
• Use your triage and clinical skills to support care staff in managing clients at their aged care facility.
• Work independently and as part of a team to improve older people's health outcomes.

Based in Gippsland, Latrobe Regional Health are looking for a Residential In Reach (RIR) Care Coordinator to join our Health Independence Program on a part time basis. You will be part of a team providing face to face and virtual support to Latrobe City Aged Care facilities to assess residents and develop plans of care with their treating team for improved outcomes and reduced hospital presentations.

We are looking for candidates with:

• Registered Nurse Division 1
• Experience in acute nursing field of at least 5 years
• Excellent interpersonal skills and the ability to work well as part of a team
