The Community Health Social Work - North team delivers evidence based Social Work services to regional and community-based clients in Northern Tasmania, who are affected by biopsychosocial issues.The service aims to provide a holistic model of care that is evidence-based, trauma-informed and locally sustainable. Practitioners within the service aim to build client capacity and resilience to self-manage and self-regulate their own health issues, as well as support the development of and participate in community driven health programs, groups and partnerships.This position is based in the Launceston area.The role:
- Within a primary health care model and in accordance with Hospitals North and Department of Health (DoH)policy and direction, legal requirements and professional competencies, provide a trauma-informed, professional social work service to individuals, families, groups and communities that are sustainable and reflective of community needs.
- As a senior member of the health care team provide support to management in developing, implementing and monitoring policies and practices that reflect the priorities and standards of the service.
- Promote community awareness in relation to building community resilience and work with other agencies, organisations and the wider community to develop an integrated approach to delivering support services.

- Please note that access to salaries beyond $115,573 is subject to qualifications and/or application to the personal upgrade scheme
- Salary range is in accordance with Allied Health Professionals Public Sector Union Wages Agreement 2022

- Full member of Australian Association of Social Workers
