About the role

We are looking for STRC Coordinator for the Western Corridor of the Central Home Care Region based at Cannington.

In this role as a Short-Term Restorative Care Coordinator, you will be negotiator and an advocate for our clients within our program to ensure the best possible outcome.  You should also be able to manage cases and help with program development in line with STRC program guidelines. You will also need to ensure data collection is reported and evaluated to determine the effectiveness of the programs.

You will be located at Cannington but will be required to travel in the community as well. You will be joining a fun and supportive team who cares and works to ensure clients receive the best care to live independently at home.

About you

This role will be perfect for someone who has:

  • Demonstrated approximately 3 years of experience in case management or client assessments.
  • Demonstrated knowledge of service provision, allocation, and compliance with contractual obligations to ensure quality service.
  • Well-developed communication and interpersonal skills
  • Demonstrated time management and organisational skills to effectively plan and prioritise work tasks
  • Demonstrated empathic and flexible approaches to manage the sensitivities of conflict resolution of consumer / employee issues
  • Proficient in the use of the Microsoft suite of programs including Outlook, Word and Excel with experience using Information Management Systems
  • Registered Nurse or Allied Health degree or equivalent Case Manager experience
  • Current C Class Driver’s Licence
  • National Police Clearance (obtained within the last six months)
  • COVID and Influenza vaccinations are preferred but not required.
