About the Role: 

We are seeking a dedicated and experienced Aged Care Worker to join our Home Care team. This position is ideal for individuals with a higher qualification who have a passion for working with clients with complex care needs and who are committed to providing exceptional care and support. The successful candidate will have the opportunity to work closely with our more complex clients while also serving as a mentor and support system for lesser qualified Home Care staff.
 

 In this role, You will:  

  • Provide high-quality care and support to clients with complex care needs, including but not limited to individuals with physical disabilities, chronic illnesses, dementia, and mental health challenges
  • Contribute to implementation of personalised care plans and evaluation of clients goals 
  • Assist clients with activities of daily living, including bathing, dressing, grooming, medication management, and meal preparation, transfers, and domestic support
  • -Monitor and assess clients' health and well-being, reporting any changes or concerns to the appropriate parties
  •  Provide emotional support and companionship to clients, promoting social interaction and engagement
  • -Mentor and provide guidance to lesser qualified staff, including Home Care Workers and Domestic Support Workers
  •  Support the training of Home Care staff to enhance their skills and knowledge
  • Collaborate with Case Managers and other healthcare professionals, including nurses, therapists, and physicians, to ensure coordinated and comprehensive care for clients

About you:

You are passionate about what you do and want to make a difference in the lives of our residents/clients, enhancing their self-esteem and empowering them to make choices.

This is the place for you if you have:

  • Certificate IV in Aging Support or equivalent qualification.
  •  Minimum of 3 years of experience working in a community Home Care setting, preferably with clients with complex care needs.
  • Demonstrated ability to provide compassionate and person-centered care.
  • Strong communication and interpersonal skills.
  • -Ability to work independently and as part of a multidisciplinary team.
  • Experience in mentoring and training staff is highly desirable.
  •  Valid driver's license and access to a reliable vehicle.
  • Comprehensive knowledge in community care with a commitment to high standards of care for clients and their families
  • Understanding of client directed care

  • A current Working With Children Check 
  • Current and ongoing flu and COVID-19 Vaccinations
  • Australian working rights
  • Ability to pass a national police clearance
