About the role

The Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have NDIS plans and/or TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model. The FFS program provides centre based and tele-health services, outreach services, support coordination, and education and vocational support. The service offers clients choice and control over services so they are able to build life skills and capacity, develop social connections and create community linkages to identify and achieve their recovery goals.

We are seeking a self-motivated, compassionate Specialist Support Coordinator for a fixed-term, part-time (30.4hours/week) opportunity to work alongside a dynamic team in South Australia. Working from home, you will provide Level 3 Support Coordination to assist participants to address barriers and reduce complexity in the mainstream and community supports, helping them to overcome any obstacles to the implementation of their NDIS funded supports. In this role you will apply the principles of choice and control by ensuring awareness and options of services are offered to the participants.

Key responsibilities

• Deliver a high level of support coordination to identify and overcome barriers though the development of a complex Support Plan, outlining risks and obstacles to effective implementation.
• Provide an exceptional customer experience ensuring service delivery needs and wants are met by external service providers within funding budgets.
• Resolve service delivery issues including crisis and ensure participant needs are identified, understood and appropriate services are in place.
• Support participants to develop skills and build capacity to understand and manage their NDIS plans.
• Collaborate with a range of services, families and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
• Demonstrated experience working in a Specialist Support coordination role.
• Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
• Proven capacity to plan and prioritise meeting customer service delivery requirements.
• Demonstrated understanding of NDIS including the NDIS price guide and legislation, with ability to build strong relationships with service providers, clinical teams and key stakeholders.
