Job description

TeamHEALTH has been providing services to people with mental illness in the Northern Territory for over 30 years. TeamHEALTH was established by a group of relatives and service providers of people with a mental illness who wished to provide stable, safe, accommodation for their family members.

TeamTALK is a low intensity psychosocial phone-based support line established for Territorians. Following the PHN Low Intensity Mental Health Services (LIMHS) for Early Intervention Guidelines TeamTALK is funded by the Northern Territory Primary Health Network (NTPHN).

Reporting to the Coordinator the TeamTALK Coach provides phone-based support to participants utilising an early intervention and psychosocial approach. The TeamTALK Coach will focus on providing supports that are person centred, recovery focused, strengths based, timely, culturally appropriate and assist callers in navigating supports and services at TeamHEALTH and in the wider community.

The TeamTALK Coach will be responsible for ensuring quality phone-based supports to both one off callers and on-going scheduled participants. Coaches will typically monitor/support participants for up to six weeks through their mental health journey.

The TeamTALK Coach will utilise a range of tools including the TeamHEALTH Stay Strong plan, a specialised Individual Recovery Plan (IRP) that enables participants to set goals. IRP goals support the development of skills to maintain independent living, self-management of one’s mental illness and service access in the community.

At times the TeamTALK Coach may be required to work across programs within TeamHEALTH.

The successful applicant will be required to have a current National Police Certificate, NDIS Worker Screening Clearance, NT Driver’s Licence and completed the NDIS Worker Orientation Module.
