An exciting opportunity exists for a dedicated Transition Discharge Coordinator to join our team at Western Health's Mental Health Service.In this role, you will play a crucial part in managing and supporting clinical activities related to patient flow at Sunshine Mental Health and Wellbeing Centre, Unit 3.Your responsibilities will include overseeing access and flow, establishing clinical standards at transitional points of care, ensuring effective resource utilisation, and providing clinical leadership and guidance to the multidisciplinary team.
