The Opportunity
As a Transition Case Worker in our Extended Reintegration Service (ERS), you’ll be at the heart of a high-impact team. You’ll use holistic outreach case management to provide specialist transitional support for people with complex needs who are leaving NSW Correctional Centres.
Using a throughcare model focusing on the complex transition from incarceration into housing in the community, your role includes:
Client support: Holistic and intensive outreach casework and support will make up a large part of your day. You’ll actively build positive relationships with Government, community and other services to source relevant support and work collaboratively with relevant stakeholders. You'll assess new referrals, complete pre-release case planning, and liaise with key stakeholders. You’ll approach every task with empathy, keeping our clients’ goals front and centre.
Creating community connections for clients: Using an outreach model pre- and post-release from custody, you’ll play a vital role in providing clients with practical and psychosocial support. The goal? To ensure connections to support services, the maintenance of tenancies, reducing recidivism, and creating pathways away from the criminal justice system.
Data, administrative and assessment duties: While the majority of this job involves client-facing duties, data collection will be an important part of your role. You’ll conduct general administrative duties, Workplace Health and Safety monitoring, and recording and maintaining accurate case notes and client files.
Cultural safety: You’ll work in culturally appropriate ways in recognition of the unique cultural needs and over-representation of First Nations people in the criminal justice system.
Your usual work hours will be three days per week, between Monday to Friday, with occasional out-of-hours work, as required.

To join us, you’ll need:
- Experience and skills in the provision of trauma-informed care
- Knowledge and casework experience working with complex and compounding needs such as homelessness, mental health, intellectual disability, criminal justice system involvement, alcohol and other drug use, and experiences of trauma
- A current NSW Driver Licence
- A current NSW Working with Children Check and Criminal Record Check (please note, a criminal record is not a barrier to being considered for a role with CRC. However, this position requires a criminal record check as part of seeking access to prisons as an Authorised Visitor)
- Demonstrated cultural competency and knowledge of issues relating to Aboriginal and Torres Strait Islander and CALD communities
- The unrestricted right to work in Australia
