Coomealla Health Aboriginal Corporation (CHAC) are seeking a General Practitioner to join their busy Aboriginal Medical Service. Based in Dareton, Far South West NSW, CHAC delivers nearly 15,000 episodes of care per annum to just over 2000 regular clients, 70% of whom are of Aboriginal and or Torres Strait Islander descent.
The work environment is challenging but rewarding and the GP is well supported by the CHAC multidisciplinary team who support with all aspects of primary care delivery.
CHAC currently have 2 full time clinics (1 in Dareton NSW and 1 in Wentworth NSW) and a prospective 2 days per week outreach clinic in Buronga NSW.CHAC has a fully accredited RVTS, ACCRM and RACGP Supervisor on site.
About the role:Our patients are at the center of our model of care, and are supported by the General Practitioner, Aboriginal Health Practitioner and Registered Nurse to participate in and plan their care, and to access a variety of primary care services that are on offer at CHAC, with an emphasis on utilization of Aboriginal Health Checks, GP Management Plans, Team Care Arrangement and Cycles of Care to facilitate good management of complex chronic conditions.
Our remuneration package is attractive, with the General Practitioner on an hourly wage as an Employee, as well as access to salary packaging benefits, a fuel card, additional break of 10 business days over Christmas.
