Where you'll be working
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing
The Vulnerable Families Coordinator position is a partnership position between WSLHD and DCJ which enables progression of a range of key initiatives in areas of mutual concern including prevention and early intervention for at risk and vulnerable children, young people and their families.
The Vulnerable Families Coordinator:

  • Provides a central point of contact for DCJ-Community Services for at risk and vulnerable children and young people who have identified health needs
  • Manages internal and external referrals for children and young people identified by DCJ-Community Services as requiring health intervention
  • Supports the implementation and ongoing management of the Pregnancy Family Meetings Project across WSLHD
  • Supports the implementation of other key partnership initiatives including Making a Safe Home (MaSH) and the Whole Family Team Project across WSLHD
  • Supports other initiatives as they evolve from the partnership with DCJ or through Western Sydney Service Delivery Reform which is the overarching auspice for these initiatives
  • Works across WSLHD and DCJ to identify service gaps and improve system responses to at risk and vulnerable children, young people and their families
The Vulnerable Families Coordinator manages the Pregnancy Family Meeting Coordinator and the WSLHD KEYS Navigator.

Selection Criteria: * Relevant tertiary qualifications in a health-related discipline and / or experience relevant to the role
  • Demonstrated experience in the delivery of health services to at risk and vulnerable children, young people and their families
  • Demonstrated understanding of child protection policy and practice and the needs of at risk and vulnerable children, young people and their families, including the roles, responsibilities and functioning of health and human service agencies and non-government organisations involved in meeting these needs.
  • Demonstrated experience in project coordination and management, including the planning, implementation, monitoring and evaluation of project deliverables.
  • Demonstrated ability to communicate, consult and negotiate effectively with senior executives, management, health staff, DCJ staff and external stakeholders
  • Demonstrated effective planning, organisational and problem-solving skills and ability to work in a changing environment, to prioritise workload and meet deadlines
  • Excellent interpersonal skills
  • Current Driver’s Licence valid in NSW
