Wellbeing Health Inreach Nurse

Where you’ll be working
Hunter New England Health are partnering with NSW Education to improve the wellbeing and health of children and young people at school (K to 12). We are advertising a new Wellbeing & Health In-reach Nurse (WHIN) Coordinator positions at Moree and Boggabilla Community Health Service. These positions work on both the health and education sites.

About the Role
The Wellbeing Nurse: Wellbeing Health In-reach Nurse Coordinator Program is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending designated primary and secondary schools. You will work in identified local schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex health care coordination for school aged children, young people and their families in these schools. The WHIN Coordinator will be working closely with school wellbeing teams, school principals, school staff, GPs, health and mental health services, child and family, youth and other specialist services, NGOs and local government services to identify health, wellbeing and social needs and coordinate appropriate intervention, assessment and referral to services for school-aged children, young people and their families. You will be supported in your role through online learning and mentoring.

About You
The Wellbeing Nurse: Wellbeing Health In-reach Nurse Coordinator role requires leadership, specialist skills, capacity for a significant level of autonomy and independent work in an interagency context. You will have extensive experience in one of the following: Paediatrics, Child and Family Health Nursing, Youth Health or CAMHS, and a related post graduate qualification (those working towards this qualification may also be considered). Coordination of access to services across multiple agencies, professions and groups will be part of the role. You will have the ability to work within an interagency context, with an emphasis on relationship building, service delivery, quality of care and team cohesion, and an ability to identify and improve opportunities to enhance integration of care for children, young people and families. You will also have excellent interpersonal and communication skills, including advanced verbal and written skills; an ability to negotiate and problem solve; and the capacity to collaborate with and influence health and other professionals.

Information for Applicants:

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
