YHP Intake and Mobile Support Worker

Full-time | 70 hours per fortnight for an initial 12-month contract, with further extension possibilities in line with funding.
6 weeks annual leave (includes 4 weeks annual leave with leave loading and 2 weeks Christmas leave without leave loading)

We are seeking a highly motivated Intake and Mobile Support Worker to join our dynamic YHP team. This position provides young people who are homeless or at risk of homelessness with solution focused crisis and early intervention Mobile Support services. This will be provided through assessment, intake, and referrals, both internal to YHP and to the wider homelessness sector and through the provision of Mobile Support prior to the young person being safely housed.
ABOUT YOU
We are looking for a passionate, client-focused individual who can work independently and within a team and can demonstrate an ability to support young people.
You will successfully
Answer calls and emails in a timely manner, providing initial response and assessment, inclusive of needs, risks, and vulnerabilities.
Respond to Walk ins on site and providing assessment and referral as appropriate.
Establish relationships with individuals who access the service, that encourage self-determination and participation.
Provide case management support to individuals in accordance with YHP case management models and standards of practice.
Carry out ongoing assessment of the individuals' presenting issues, vulnerabilities and needs from initial intake to exit.
Attend allocation and vacancies pending meetings with other Support Practitioners on a fortnightly basis to discuss pending client’s needs and properties soon to become available.

To be successful in your application you will need to provide written responses to the following selection criteria:
1. Demonstrated knowledge, or demonstrated ability to gain knowledge, of the issues and barriers experienced by young people generally and specifically in relation to housing.
2. Demonstrated knowledge, or demonstrated ability to gain knowledge, of the youth housing sector, community housing & other housing options for young people.
3. Ability to engage and work with a diverse range of young people.
4. Understanding of social justice principles and the ability to incorporate these principles into your work practice with young people.
5. Ability to develop and maintain effective links with both government and non-government organisations to promote and advocate for young people’s issues.
6. Ability to work effectively at an individual and team level as well as within a community managed organisation.
Required:
- Relevant experience and/or a degree in Social Work or the Social Sciences/Human Services, or be completing study in a relevant field, minimum of Diploma in Community Services.
- Familiarity with computer systems and software applications.
- A current driver’s license.
- The successful applicant will be required to provide a Working with Children Check -Blue Card from the Blue Card Services, Justice Services Department of Justice and Attorney-General.

ABOUT YOUTH HOUSING PROJECT ASSOCIATION INC.
Under its Crisis Accommodation Program, Youth Housing Project Association Inc. (YHP) provides supported, unsupervised, medium-term accommodation to young people aged from 16- 21 years old who are homeless or at risk of homelessness. Singles, couples, siblings, pregnant and parenting young people are able to apply for housing. Couples with a partner aged up to and including 25 years are housed under the condition that the partner accepts support and is willing to work with an individual YHP support practitioner. YHP does not offer emergency accommodation.
YHP offers support services designed to assist homeless young people to make the transition to an independent, self-managed lifestyle. In doing so, YHP acknowledges both the structural inequities that lead to homelessness and the hardship and distress that individuals who experience the process of becoming homeless are compelled to endure.
Desired outcomes are the product of informed consultation with individual young people. Supported accommodation is provided in the configuration of one- and two-bedroom units which are furnished with basic necessities. These units are located in various inner northern suburbs. These locations emphasize accessibility to public transport, employment, health, education, and Centrelink services as well as access to shopping and recreational facilities.
Support services are provided by the Youth Housing Support Practitioners based at a central office in Albion.
Under its Community Housing Program YHP provides transitional accommodation to young people exiting CAP services who are ineligible or unable to obtain Private rental and meet the Dept of Housing’s eligibility criteria. These young people are deemed ready for more independent living. Minimal support is provided by the YHP Support Practitioners.
