About the role
We are seeking a dedicatedYouth Case Managerfor our Housing and Homelessness program! You will deliver high quality case management support services to young people including families and accompanying children who are at risk of or experiencing homelessness.
Responsibilities include
- Identify clients who are displaying symptoms of mental illness and respond as per policy and procedure
- Defuse and manage situations where a resident is displaying inappropriate and/or anti-social behaviour
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Case management support provided to clients
- Provide expert input to influence industry or sector development
This is a fixed-term full-time position, contracted until June 2025, based in Shepparton, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
You will have
- Tertiary qualification in Social Work, Welfare, Community Development or related field
- Diploma is recognised with 5+ years' experience and ability to study towards tertiary qualifications
- A minimum of three years' experience in a social service environment preferably the youth sector
- A working knowledge of the challenges facing young people in today's society such as alcohol and other drugs/homelessness and impact this may have on physical and mental health
- A current Driver's Licence
- All mandatory training including TSA specific training completed and up to date
