About the Role
- Fulfil the accountabilities and responsibilities of this role in accordance with Metro South's purpose and objectives, as outlined below.
- This position, collaborating with members of a multidisciplinary team, will:
- Support and liaise with hospital and community Chronic Disease services to coordinate health care service delivery and information for Aboriginal and Torres Strait Islander people and their families.
- Link Aboriginal and Torres Strait Islander people and their families to appropriate local health and social service providers.
