 What you’ll be doing in this role?

As an Aged Care Assessment (ACAS) Clinician, you will undertake comprehensive assessments in the community, residential care settings and regional hospitals. You will support clients in establishing wishes and goals to either remain living in the community or move into residential care. You will find an incredible sense of workplace satisfaction and achievement throughout a role which is both challenging and rewarding.

Your work will include:

  • Conduct comprehensive holistic assessments, in the community, residential care settings and at regional hospitals of client’s needs resulting in a care plan that takes into account the client/carers desire to either remain living in the community or to move into residential care.
  • Conduct assessments for people from vulnerable groups, including those from a culturally and linguistically diverse background (CALD) and an indigenous backgrounds.
  • Manage client database and documentation recording across various platforms and information technology.
  • Undertake and complete Support Plan Reviews as per current work procedures and ACAS guidelines. 

What we’re looking for:

  • Bachelor Degree in Social Work, Occupational Therapy, Nursing or Physiotherapy.
  • AHPRA registration or equivalent professional membership/registration
  • A current Victorian Drivers Licence
  • A current Working with Children Check (Employee)
