Our services include general practice (GP); nursing; allied health; dental; counselling and psychology; gambling support; alcohol and drug treatment; migrant and refugee settlement services; social connection; carer support services and National Disability Insurance Scheme (NDIS) in partnership with the NDIA.What you'll be doing in this role?Reporting to the Manager Gateway, provide day to day leadership and management support to the Aged Care Assessment Service (ACAS) and Regional Assessment (RAS) teams. The ACAS Team leader takes pride in mentorship, and supports the solid foundation of positivity and optimism in the attitudes and behaviours of team members.The ACAS Team Leader supports a safe and uplifting environment that will constantly motivate staff to bring their best to work to achieve and learn. The ACAS Team Leader supports the Gateway Manager by contributing expertise and experience in planning and quality improvement projects.Your work will include:

• Coordinate and provide leadership to the ACAS and RAS service across Gippsland.
• Manage and monitor daily workflow demands and provide operational advice to the Manager Gateway in order to meet demands.
• Conduct referral intake processes as per current work procedures and ACAS/RAS guidelines, including the triage of referrals received by the My Aged Care (MAC) portal and other referral pathways.
• Monitor and manage team performance against monthly and annual program targets in collaboration with the Gateway Manager.

What we're looking for:

• Bachelor Degree in Social Work, Allied Health or Nursing
• Previous leadership experience with a multidisciplinary team including conducting Annual Performance Reviews
• A current Victorian Drivers Licence
• A current Working with Children Check (Employee)
