About the Opportunity
South Western Sydney Local Health District is recruiting for a Health Clinician (Level 1/2) to complement and enhance the current Aged Care Assessment Teams (ACATs).

What You'll be Doing
Aged Care Assessment Teams (ACATs) play a pivotal role in the Aged Care Assessment Program. The focus of ACAT work is to comprehensively assess the care needs or frail, older people. In doing so, they assess all the care needs of a person using a multi- disciplinary and multi-dimensional approach. The ACAT Assessor is an active member of the multi-disciplinary ACAT. Assessments are conducted for residents of SWSLHD and for people who are inpatients of hospitals in the SWSLHD.
A key function of this assessment process is the determination of the client's eligibility for approval to access aged care services as set out by the Aged Care Act (1997).

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

Relevant qualifications as a Registered Nurse, Occupational Therapist or Physiotherapist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA), or hold a bachelor or post graduate degree in Speech Pathology or Social Work which provides eligibility for membership of the relevant association, or other qualification deemed equivalent by the employer.
Proven high standard of written and verbal communication skills.
Demonstrated knowledge of the role of the Aged Care Assessment Team, aged care programs and services and recent/relevant aged care reforms.
Demonstrated post graduate experience working with Aged Care Clients and proven ability to undertake comprehensive assessments and formulate, and implement appropriate care plans for older people in community and /or inpatient settings.
Proven clinical skills in the assessment of dementia and other age related diseases
Proven ability to work independently as a clinician and collaboratively as a member of multidisciplinary teams and to work flexibly across and within all ACAT sites.
Proven ability to use Microsoft Office Programs, electronic patient record systems and e-learning sites
Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
