Our network of small local teams enables Chorus people to be more connected to each other and feel a sense of belonging.Position OverviewThe Access Enabler Mentor is crucial in empowering and improving the lives of customers with psychosocial disabilities. As you assist the customers in navigating the NDIS access process, you will ensure they understand what the process entails and assist them in applying for NDIS funding. You will also conduct assessments, complete the Evidence of Psychosocial Disability Form, and provide ongoing support for customers to access necessary services and relevant support such as therapy or counselling support.Key Responsibilities:
  • Conduct initial assessments to understand the customer’s psychosocial disability and support needs.
  • Engage with the customer, their families, and caregivers to build trust and gather comprehensive evidence and information relevant to their NDIS access request.
  • Assist customers in preparing and submitting NDIS access requests, ensuring all necessary documentation and evidence are included.
  • Coordinate with healthcare professionals to obtain relevant assessments, reports, intervention reports, discharge summaries and other supporting documents required for the customer’s NDIS applications.
  • Provide customers with clear and accurate information about the NDIS, including eligibility criteria, application processes, and available support.
  • Advocate on behalf of customers, ensuring their needs and preferences are not just heard, but accurately represented in their NDIS applications and planning meetings. Your role is crucial in making sure their voice is the guiding force in their NDIS journey.
  • Liaise with the Local Area Coordinators (LACs), also known as Community Connectors. They are the key point of contact for all customers wanting to access. Facilitate community connection meetings on behalf of customers and support customers in attending the meeting for their NDIS application submissions.
  • Maintain accurate and up-to-date records of customer interactions, assessments, and progress.
  • Prepare regular reports on customer outcomes and service delivery for internal and external stakeholders.
  • Support customers in understanding their NDIS plans and how to implement them effectively.
  • Provide ongoing support to customers post-access approval to transition customers to relevant external stakeholders, ensure they utilise their plans effectively, and access all entitled supports.
Qualifications and Experience:
  • A qualification in Social Work, Psychology, or a related field.
  • Experience working with individuals with psychosocial disabilities.
  • Knowledge of the NDIS, eligibility criteria, application processes, and available support in Bunbury or South West Region.
  • Excellent communication, interpersonal and communication skills, with the ability to engage effectively with customers, families, and other professionals.
  • Excellent organisational skills and attention to detail.
  • Ability to work independently and as part of a team.
  • A valid driver’s license and access to a reliable vehicle.
Desirable:
  • Previous experience in a similar role within the mental health sector.
  • Understanding of mental health issues and their impact on individuals and families.
  • Familiarity with local support services and resources available to individuals with disabilities.
  • Strong organisational and time management skills with the ability to manage multiple tasks and priorities.
  • Empathy, patience, and a genuine commitment to supporting individuals with psychosocial disabilities.
  • Strong advocacy skills with the ability to navigate complex systems and processes.
Working Environment:
  • Office-based with regular outreach and customer visits in their home or the community.
  • Some travel may be required to meet with customers and attend meetings or appointments with customers.
