East Grampians Health Service is seeking application for ACAS Assessor to join our team. This is a temporary (May 2024 – December 2024) part time position of 8 hours per fortnight.
East Grampians Health Service is based in Ararat and Willaura (Regional Victoria) and takes its name from the magnificent mountain range that provides a backdrop to both townships. Community Nursing is a part of the broader Community Services Department based in Ararat. We provide community nursing across the catchment area of Ararat Rural City. Community nursing includes palliative care, district nursing, hospital in the home, hospital admission risk program (HARP), home care packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.

This role provides comprehensive assessments to ensure aged community members receive the care and support that is needed.
About YouThe successful applicant will have;
- Demonstrated sound level of clinical assessment skills
- Demonstrated experience in a ACAS role
- Demonstrated ability to contribute and practice collaboratively as part of a multidisciplinary team
- The ability to work independently and within a team environment
- Be committed to best practice and have skills in problem solving
Appointment is subject to immunisation clearance (Including COVID-19) and current and satisfactory Employment Checks (Police and Working With Children Checks)
