Are you an experienced, passionate Case Manager looking to join a dynamic, established and growing Home Care Provider? 365 Care is looking for just that person to join our Aged Care team in Penrith, New South Wales. The successful candidate will be responsible for providing comprehensive case management services to our elderly clients in the community, ensuring the delivery of high-quality care and support throughout their Aged Care journey. This role requires excellent communication skills, strong attention to detail, and a compassionate approach to client support and engagement.
No two days are the same! Key responsibilities are varied and include elements of a case management role such as assessments of clients needs and the development of individualised care plans in collaboration with healthcare professionals and client families. This role will also monitor clients' progress regularly and make necessary adjustments to the care plan as needed and provide guidance to clients and their families regarding their home care package. To excel in this role, you will enjoy collaborating with varied stakeholders and, be comfortable with the administrative elements of case management in compliance with regulatory standards.
Key selection criteria includes:
- Proven experience as a Case Manager within a community setting (clinical experience will be highly regarded)
- Confident communication and interpersonal skills with the ability to build rapport quickly with clients from diverse backgrounds
- Strong organisational and time management skills with the capability to manage multiple tasks simultaneously while maintaining attention-to-detail and accuracy
- Exceptional written and verbal communication skills
- Knowledge of the Australian Aged Care system, care planning processes, and home care regulations
- Proficient computer literacy, including experience using electronic record systems.
