The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.
We are looking for an Aged Care Coordinator who will assume a key role in the coordination and case management of aged clients considered for or within the Transition to Care Program (TCP). The coordinator will develop collaborative relationships across the health service and community programs to provide high quality case management. The Aged Care Coordinator will support the timely management of TCP clients as they complete their restorative process, optimise their functional capacity, finalise, and access their longer-term care arrangements.
This role will see you providing leadership to a passionate team and subcontractors, monitoring KPI’s and identifying gaps in skills and knowledge of the team. You will continue to develop a cohesive and successful team.
Specific requirements of this position include but are not limited to;
Essential Criteria
- Relevant Tertiary qualifications and Aged Care experience.
- Demonstrated knowledge of issues affecting patients with complex care needs.
- Experience in undertaking complex assessment and care coordination of client in a community setting.
- Demonstrated experience managing and leading a team within a culturally diverse workplace.
- An understanding of the unique issues affecting and impacting on the health and wellbeing of Aboriginal and Torres Strait Islander people.
- Highly developed interpersonal skills including negotiation and consultation skills and the ability to proactively establish and sustain effective stakeholder relationships.
- NDIS Clearance or willingness to obtain
- WA Drivers Licence
