Under the proven model of Care, General Practitioners access RACFs through existing relationships. The model is based on a year-round clinical health service plan and processes, aiming to provide comprehensive healthcare for your Aged Care residents. You will have access to nurse practitioners to assist with your patients' clinical needs, as desired, allowing you to see more residents.
