Northern Adelaide Local Health Network (NALHN) is eager to recruit an Occupational Therapist or Physiotherapist to work in our dynamic, growing and supportive team, to help us deliver exceptional care to our community.

The network boasts newly redeveloped wards and Emergency and Outpatient Departments, with plans for the addition of more brand-new facilities in the near future. Our 300-plus Allied Health professionals play an integral role in multidisciplinary teams delivering outstanding care to patients across our acute, subacute and hospital avoidance services.

About the Role:

An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as an Allied Health Acute Aged Care Coordinator within the Allied Health Division.

The Allied Health Acute Aged Care Coordinator is accountable to the Director Clinical Operation Allied Health NALHN for:

  • Providing an expert clinical complex / case coordination / consultative service to wards, units and departments in facilitating integrated assessment, care planning, service delivery and discharge coordination of eligible aged patients.
  • Working with the broader nursing, medical and Allied Health treating team to facilitate early identification of aged consumers who would benefit from comprehensive care planning inclusive of behaviour management, mobility and diversional programs.
  • Act as a NALHN aged / dementia / behaviours of concern expert, inclusive of building effective working relationships with relevant community agencies their partners, relevant non-government organisations and the Department of Health and Wellbeing.
  • Monitor and escalate client system issues and discharge delays whilst working with the broader nursing, medical and Allied Health treating team to support acute inpatients and facilitate a smooth transition to residential care facilities complex situations and backgrounds requiring time limited case coordination, comprehensive assessment, and tailored supports to achieve a safe and sustainable discharge, avoiding unnecessary hospital representation.

In this role you will be an expert within your area and stream of practice and have a significant degree of autonomy and decision making.

To be eligible for this position, you must hold:

  • Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with Occupational Therapy Australia OR
  • Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.

Appointment will be subject to a satisfactory Criminal History Check.

  • National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. Aged Care employment or work involving vulnerable clients)
