Your new role
Wintringham is seeking a dynamic and proactive Area Coordinator, to join our multi-disciplinary community team on a Full-Time basis. This leadership opportunity will provide support and supervision for our team of Case Managers and Care and Support Workers to make sure that clients receive the support and care needed based on their funded level of package through the Home Care Package Program (HCP) or Commonwealth Home Support Program (CHSP).
The role will provide case management to a small number of clients as well as having a key focus on program establishment and growth. This role will work closely with the on-site Housing and Homelessness Team to assist Wintringham clients to access aged care supports and services that they need. Services will be targeted to the Wintringham clientele and a key goal will be to ensure that clients living in our Housing Program receive appropriate care and support services to remain living at home in the community of their choice.
Your duties and responsibilities will include:
- Promote a non-institutional philosophy by recognising and promoting the rights of clients
- Ensure duty of care to both clients and staff is adhered to at all times
- Ensure high quality, comprehensive case management services are provided.
- Provide support and supervision for a team of Care and Support Workers at the Hobart office and any staff supporting clients in the CHSP.
- Oversee the growth of the Home Care Package Program in the region (Hobart and surrounding cities/suburbs) with suitable liaison with other external key stakeholders to promote our Program and intake of new clientele
- Maintain an expanding caseload of Home Care Package clients as well as oversight of the the existing CHSP clients.
We are looking for candidates with the following:
- An approved tertiary qualification in Social Work, Nursing, Welfare, Community Services or Allied Health.
- An understanding of the aged care sector and community services
- Current Driver’s License and reliable motor vehicle with capacity to transport elderly clients and their mobility equipment
- Experience in case management of clientele, supervision of staff and staff development
- Experience in service delivery in a community setting
- Sound understanding of the aged and community services legislative frameworks
- Experience in the Health or the Aged Care Sector and Homelessness
- Commitment to the organisation and its philosophy of social justice.
- Well-developed communication and liaison skills with a variety of people from varying socio-economic backgrounds
