Hands On People an Official NDIS Provider supporting Territorians living with disability and assisting them to navigate the NDIS(The National Disability Insurance Scheme) and get support faster.
Our team provides support services and expert advice to help transition our participants to independent living.
Our services include Accommodation Services, Community Nursing, NDIS Support Coordination, NDIS Plan Management, Social and Community Participation, Veteran’s Home Care and more.
Our primary services operate out of our two main office locations - Darwin & Alice Springs.

Roles & Responsibilities:
- Schedule and organize engagement programs for our participants and communicate the activities with the team
- Managing Participants files – Documentation, ensuring SA’s are up to date, following up unsigned Service Agreements with participants and COS for community access.
- Assist in and Coordinate, plan, and execute workflow activities including maintaining a professional relationship with participants, families, and other stakeholders.
- Ensure all participant reports meet, compliance and reporting requirements, of the NDIA and NDIS Quality and Safeguarding Commission and ensure case notes and other documentation of our participants are up-to-date, and accurately compiled.
- Attending sign-ups/ meet & greet for new Care Services participants.
- Coordinate, plan, and execute workflow activities including maintaining a professional relationship with participants, families, and other stakeholders.
- Provide mentorship, support, and supervision for a team of disability support staff and House Leads
Required Knowledge, Experience and skills
- Certificate III/IV in Disability or Individual Support.
- Prior relevant experience as Support worker or Team Lead preferred
- Current NT unrestricted driver’s license
- Current First Aid Certificate and CPR
- Valid OCHRE card.
- Excellent interpersonal, communication, and resilience skills
- Adaptability in being able to deliver person-centred services in line with participant goals and needs
